Design Failure Mode and Effects Analysis (DFMEA)

In today’s environment of rapid product development, compressed timelines and increasing customer expectations, organisations can’t afford to discover critical design weaknesses late in the lifecycle. Design Failure Mode and Effects Analysis (DFMEA) provides a structured, proactive approach to identifying and reducing risk during the design phase.

Introduction

Unlike Process FMEA, which focuses on failures introduced during manufacturing or delivery processes, Design FMEA concentrates on the product or system design itself. It examines how a design could fail to meet functional, performance, safety or reliability requirements, enabling teams to address issues early when changes are less costly and easier to implement.

This practical programme provides the skills to apply DFMEA effectively within real product or service development environments. Participants learn how to identify potential failure modes, evaluate risk, prioritise actions and facilitate structured cross-functional reviews that strengthen design robustness, improve quality and support faster, more confident innovation.

The course suits anyone facilitating or participating in DFMEA activity, including programme and product managers, design engineers and cross-functional team members.

Participants will be able to:

  • Understand essential DFMEA concepts and terminology
  • Follow the step-by-step DFMEA process flow with confidence
  • Apply methodologies for risk assessment and mitigation within design
  • Develop effective facilitation strategies for leading DFMEA sessions
  • Apply DFMEA concepts to real-world scenarios and case studies
  • Integrate DFMEA with related core quality tools and build action plans

Why choose this programme?

DFMEA enables organisations to identify and address potential design weaknesses before they become costly manufacturing issues, field failures, service failures or customer complaints. By embedding risk thinking early in development, teams improve reliability, safety and performance and launch readiness for their products and services, and they reduce rework, delays and cost. The programme carries 16 CPD points.

Delivery Information / Options

The programme is delivered in-company, either virtually or on-site, and can be tailored to align with industry or in-house standards and tool templates. Practitioner training runs over two days, with a one-day option for leadership awareness, team awareness or practitioner refresh training. Sessions are bespoke and highly practical, with delegates encouraged to work on their own live examples in classroom exercises.

Course Overview

The programme introduces the principles, terminology and application of Design FMEA before guiding delegates through the full DFMEA process, including defining scope, identifying failure modes and effects, analysing causes, applying prevention and detection controls, assessing and prioritising risk, and developing effective mitigation actions. Participants also build facilitation skills for leading productive cross-functional DFMEA sessions and explore how DFMEA integrates with wider quality and product development activities. Throughout the programme, delegates apply the methodology to practical examples and live organisational challenges to support immediate workplace application.
